Advance Directive 

  • The Federal Patient Self-Determination Act of 1990 describes your rights to accept and/or refuse treatments. The Federal Act requires all health care providers who accept Medicare or Medicaid to give you information, ask if you have an Advance Directive and document those answers. If you have an Advance Directive, someone here will place a copy in your medical record.

  • If you have a Living Will and/or Durable Power of Attorney for Health Care, please provide a copy to be filed in your medical record.

  • If you would like more information concerning an Advance Directive or Durable Power of Attorney for Health Care you may call 6-3636 from your phone or visit the Admissions department.
